1) Purchase of one hundred (100) Developmental Assessment of Young Children 2nd Edition completely testing protocol sets The DOH, Early Intervention Section (EIS) is seeking a vendor to purchase one hundred (100) Developmental Assessment of Young Children – Second Edition complete testing protocol sets. A complete testing protocol set includes one each (25 forms packets) adaptive, cognition, communication, motor, and social. Testing protocol sets are needed as an alternative evaluation instrument to determine eligibility of children referred for IDEA, Part C services. 2) Drop shipment of one hundred (100) Developmental Assessment of Young Children 2nd Edition testing protocol sets to 18 geographical locations throughout State of Hawaii The vendor must be able to drop ship the one hundred (100) complete testing protocol sets (i.e. one 25 pack each of adaptive, cognition, communication, motor, and social) to each of the 18 locations across the State of Hawaii and meet vendor compliance requirements as specified by the State of Hawaii. Vendor will fulfill EIS purchase of one hundred (100) complete testing protocol sets and drop ship testing protocol sets as specified in the attached spreadsheet. Vendor will drop ship testing protocol sets in accordance with purchase and distribution detailed in the attached spreadsheet. Specific Hawaii drop ship locations, addresses, number of testing protocol sets, and complete testing protocol sets for each location can be found in the attached spreadsheet.
