Oak Street, Scottsdale, AZ 85257-1101 for Blood Donor Testing in support of our Blood Bank located in the Laboratory Department of U S Naval Hospital, Guam. Utilization of Barcode ISBT-128 is required. NAICS Code 621991 / 621511. This requirement is procured in accordance with FAR PART 13. This is NOT a request for proposals.
