The U S Naval Hospital, Guam intends to negotiate a Firm Fixed Price standing order type contract on a sole source basis with Creative Testing Solutions,  6210 E. Oak Street, Scottsdale, AZ   85257-1121 for Blood Donor Testing in support of our Blood Bank located in the Laboratory Department of U S Naval Hospital, Guam.  Utilization of Barcode ISBT-128 is required.  NAICS Code 621991 / 621511.  This requirment is procured in accordance with FAR 13.  This is NOT a request for proposals.  No solicitation document will be issued.  Determination not to compete this requirement is solely within the discretion of the Government.
