The Department of Human Services (DHS) is seeking a contractor to provide Spiritual Supports services to individuals at the Ebensburg Center. This Supplier Price Request will result in a DHS Contract for the qualified vendor to provide Non-Denominational Chaplaincy Services (other than Jewish, Catholic, and Muslim) for based on the Statement of Work. Bids are due on November 14,2022 at 4:00 PM and must be submitted electronically. Late bids will be disqualified. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder. Bidders will provide their cost by entering their per unit cost; a unit is described as EA which is equal to 1 Hour of Spiritual Support Services for the designated unit on each line for each year of the contract. Units are defined in the Statement of Work and will include ALL attendant costs including travel. Travel should be calculated in accordance with the Commonwealth Management Directive 230.10. Supplier must enter firm prices for all contracted years. - Line 1 is Year 1 and Line 2 is Year 2.
