Specific Requirements:  The Physician must maintain malpractice insurance with limits consistent with the medical service industry, but in no case less than $1 million per occurrence, worker's compensation insurance and auto insurance.  EJCA should also be provided current copies of all professional licenses, certifications and professional liability insurance documents required by the Center.  Physician must complete and submit W9 and Vendor Questionnaire.

Payment Terms:  Net 30 days

Contract is awarded based on: Price - lowest reasonable price; Quality - demonstrated through experience and knowledge; Past performance - past services to center or list of references

Schedule:  This is a 4 hour per week position; 20 weeks from July 1, through November 30, 2014.
