Request for Proposals (RFP) # 872 for ADULT DAY PROGRAM (ADP) AND ALZHEIMER’S DAY CARE RESOURCE CENTER (ADCRC) The City and County of San Francisco Human Services Agency (HSA), Department of Disability and Aging Services (DAS) announces its intent to seek proposals from non-profit organizations interested in providing (1) Adult Day Program services for older adults and adults with disabilities; and/or (2) Alzheimer's Day Care Resource Center services for individuals with Alzheimer’s disease and related dementias, and counseling, training, support and respite for their families and caregivers. The source of funding for these services is the County general fund. The grant agreement shall have a tentative term from July 1, 2021 to June 30, 2024, subject to annual availability of funds, annual satisfactory grantee performance, and need. SF-HSA reserves the right to enter into grants of a shorter duration and to make multiple awards through this procurement process. The table below estimates the annual level of funding potentially available for each service category. Service Category Estimated Annual Funding Adult Day Program (ADP) $466,224 Alzheimer’s Day Care Resource Center (ADCRC) $465,392 Payment for all services provided in accordance with provisions under this grant shall be contingent upon the availability of funds. The City does not guarantee any minimum amount of funding for these services. Applicants may elect to submit a proposal for the Adult Day Program alone, the Alzheimer’s Day Care Resource Center alone, or both Adult Day Program and Alzheimer’s Day Care Resource Center. A separate proposal package is required for each service category. This should include a description of the organization’s plans, strategies, and activities to address racial equity and inclusion among staff and program participants, as well as internal controls to regularly review current practices through the lens of racial equity and inclusion to identify areas of improvement. Additional examples of information to be provided are: the organizational mission or inclusion statements, non-discrimination documents, and/or other supporting documents, community outreach plans, staff training activities on racial equity, and a description of or data on the demographics of staff and program participants.
