Request for Proposals (RFP #874) for Aging and Disability Resource Center The San Francisco Human Services Agency (SF-HSA), Department of Disability and Aging Services (DAS) announces its intent to seek proposals from organizations or individuals interested in contracting to provide: 1. Aging and Disability Resource Centers (ADRC) to provide information, assistance and translation services at districts throughout San Francisco providing crucial supports for many older adults, adults with disabilities and caregivers in the community. 2. The Citywide ADRC Coordinator to provide training and clinical oversight to ADRC staff, as well as coordinating and executing an ADRC outreach plan. Respondents can apply for one or both grants. For grantees applying for both, separate proposals are required. HSA/DAS anticipates awarding multiple Grantees as a result of this RFP. Selection will involve providing services to all districts. Grantees will be chosen throughout the city, with a concentration in areas that have dense populations of high need and with an ability to provide services to younger adults with disabilities. It is the intent of DAS that every supervisory district is represented and/or has access to ADRC services, with 2 centers in District 6. In addition to coverage for every supervisory district, there will be selections for Citywide ADRC focused on outreach to the disabled clients. The City reserves the right to make at least one award to an organization which focuses its services specifically on adults with disabilities. Respondents should identify on the RFP Cover Page in which district(s) they intend to provide ADRC services. Respondents should also identify if they are a disability focused ADRC provider. The total available funding for this RFP is estimated to be $1,290,857 annually. Additional ADRC centers may be selected at the sole, absolute discretion of SF-HSA, based on available budget after the selection of the ADRC roles defined above. The source of funding for these services includes federal, state, and local funds. Payment for all services provided in accordance with provisions under this grant shall be contingent upon the availability of funds. The City shall not be required to provide any definite units of services nor does the City guarantee any minimum amount of funding for these services.
