RFP 904: Long-Term Care Ombudsman Services The San Francisco Human Services Agency (SF-HSA) announces its intent to seek proposals from nonprofit organizations interested in providing Long-Term Care (LTC) Ombudsman Services to older adults and younger adults with disabilities. The primary responsibility of the LTC Ombudsman Program, as required by federal and state statutes, is to investigate and resolve complaints made by, or on behalf of, residents of long-term care facilities. The purpose of this grant is to improve the quality of life and quality of care for people living in nursing homes, residential care homes, and assisted living facilities. The LTC Ombudsman Program seeks to accomplish this goal by 1) receiving and responding to complaints and issues presented by, or on behalf of residents of long-term care facilities and 2) providing consultation and education on resident rights and good care practices system wide. The total available funding for this RFP is estimated to be $722,000 annually. SF-HSA anticipates awarding one grantee as a result of this RFP. The source of funding for these services includes federal, state, and local funds. Funding for this RFP includes California Department of Aging (CDA) funds. Contractors will be required to follow CDA rules and regulations referenced in Section VII.J. Payment for all services provided in accordance with provisions under this grant shall be contingent upon the availability of funds.
