Request for Proposals (RFP) #943 for Choosing Healthy Appetizing Meal Plan Solutions for Seniors (CHAMPSS) The San Francisco Human Services Agency (SF-HSA) and the Department of Disability and Aging Services (DAS) announces its intent to seek proposals from organizations interested in providing congregate nutrition services through restaurant partnerships, also known as CHAMPSS (Choosing Healthy Appetizing Meal Plan Solutions for Seniors). DAS has allocated funding across three supervisorial districts based on the existing provision, identified community need, and key priorities as described in the DAS Service and Allocation Plan. SF-HSA and DAS will award one or more grants to provide the nutrition services described in this solicitation in the supervisorial districts identified in the table below. The table below also identifies the estimated annual funding for a CHAMPSS program by the supervisorial district and the required nutrition compliance quality assurance (NCQA) components: Estimated Annual Funding Amount Supervisorial District 4 $96,225 Supervisorial District 7 $238,638 Supervisorial District 11 $50,037 NCQA Components for Congregate Nutrition Services $15,100 Total $400,000 The total available funding for this RFP is estimated to be $400,000 annually. The source of funding for these services is local funds. Payment for all services provided in accordance with provisions under this grant shall be contingent upon the availability of funds. The City does not guarantee any minimum amount of funding for these services. A respondent may bid on one or more of the supervisorial districts. A SEPARATE proposal is required for each supervisorial district. Proposals for supervisorial district will only compete against proposals applying for the same supervisorial district. Actual funding amounts may vary depending on the number and quality of bid responses received and previous service delivery in the districts and/or identified community need. Nutrition compliance and quality assurance (NCQA) components are required to provide the CHAMPSS program. Respondent should address NCQA components in each proposal submitted and include NCQA components in corresponding budgets. All of the nutrition services sought through this procurement are for existing services. Any agency awarded a new grant, other than the incumbent, must commit to developing a transition plan to transfer existing participants to the organization’s nutrition services with the goal of minimal to no disruption of services.
