The San Francisco Human Services Agency (SFHSA) announces its intent to seek proposals from nonprofit organizations interested in providing kinship family caregiver support services for the Department of Disability and Aging Services (DAS). Kinship Family Support Services are an array of services designed to help caregivers in maintaining quality home care and improving both caregiver’s and care-recipient’s well-being. The purpose of this grant is to assist San Francisco residents who are grandparents, step-grandparents, or any other older relative of a child by blood, marriage, or adoption who is 55 years of age or older, living with the child, and identified as the primary caregiver through a legal or informal arrangement. Qualified applicants will provide services to family caregivers caring for children 18 years or younger or caring for someone with a disability. Services include, but are not limited to: community education to current and potential caregivers, outreach activities and information and assistance to caregivers; support services which include caregiver assessment, caregiver counseling, caregiver support group, caregiver training and case management; respite service which includes in-home and out of home respite service; and supplemental services such as legal assistance and assistive devices.
