The Contractor to provide Exterminating Services for the:

1.  Chinle Hospital and 3 Clinics within the Chinle Service Unit.

2.  Government Housing for Chinle. AZ

For Servies: August 2008 and September 2008.

I. INTRODUCTION SERVICE REQUESTED

Contractor to furnish all Labor, Product (Pesticide) equipment
transportation and licensure necessary to perform monthly
exterminating services of the IMS Quarters listed below.

NOTE: The contractor and/or his employees while performing the
work under this contract in government - owned or
controlled building or on Indian or government facilities
shall be subject to the same regulations, as those
governing the conduct of government employee.

II. DEFINITION
Pesticide - Chemical product used must be EPA approved. Material
Safety Data Sheet (MSDS) must be approved on all chemical products.
Rodenticide and Insecticide applied must be odorless and shall not be
harmful to pets, plants or humans.
APPLICATION Inside Pesticide must be introduced (sprayed) into
corners along, baseboards behind equipment, shelves, cracks or holes.

III. GOVERNMENT FURNISHED PRODUCT - NONE.

IV. CONTRACTOR FURNISHED EQUIPMENT.
1. Pesticide, Rodenticide and traps.
2. Spraying equipment approved for indoor used.

V. Specific Task - Spray the following facilities once every month.
1. Trailer: CL-4012 and 1 Duplex Unit at the entrance to old BIA
compound.
2. 1 Duplex building: 68.1 and 68.2 on the old BIA Compound &
storage shed.
3. 7 buildings with garages: CL-2008 to CL-2013.4 south of the
new BIA Compound. Note: 2013 duplex has a storage shed (4
each).
4. 5 duplex units on new BIA Compound: 2102.2 to 2105.2 units
have storage5. 20 buildings with garages; CL-2017 to CL-2036 west of the
Chinle IMS Hospital.
6. 161 units with attached garages: Chinle IMS Compound.
7. 13 units with attached garages: Many Farms IMS Compound.
NOTE: 4 units with storage sheds.
8. 23 units with attached garages: Tsaile IMS Compound.
9. 11 duplex (new) Chinle.
10. 2 duplex (new) at Tsaile.
11. 10 duplex Trailer

VI. Chinle Quarters - Spray inside furnace room and hot water heater
spaces to 161 IMS Quarters: CL-2037 to CL-2100 (Located north of
the Chinle IMS Hospital).

 Spray quarters exterior, along up and along foundations, along wall interface, wall openings and garages.

1. Approximately 95% of the quarters will be occupied at the time
of each services, the contractor will alter or phase work
accordingly. Services will be performed preferably on
Wednesday between 8 - 5pm. The contractor shall submit a
carefully considered work schedule to the Facility Management.
2. Contractor to cooperate fully with occupants to minimize conflict.
Problems encountered will be address to Norma Irvin at (928)
674-7303.
3. Additional spraying may be conducted while on site at any of
these location, Housing Manager (Norma Irvin) shall have the list
if any.

VII. Specific Task - Spray the following facilities once every month.
1. Chinle Hospital - 115,000 sq. ft. includes Laboratory Building.
2. Morgue Building - 400 sq. ft.
3. Facilities Management Building - 6000 sq. ft.
4. Wellness Building
5. Women's Health Building
6. Education/Administration Building
7. Hogan
8. New Pinon Health Center (2 Level) 68,000 sq. ft.
9. Many Farms Health Station (2 Level) - 12,401

10. Tsaile Health Center w/Crawlspace - 36,635 sq. ft.

VIII. The Chinle Hospital - Exterior spraying only to the area outside and
immediately adjacent to the building. Unless specifically identified
area to spray.

1. The facilities will be occupied at the time of each services, the
contractor will alter or phase work accordingly. Services will be
performed preferably on Thursday mornings when clinics are
closed for Administrative meetings. The contractor shall submit
a carefully considered work schedule to the Facility Management.
2. Contractor to cooperate fully with users to minimize conflict and
interruption of direct - patient care services. Problems
