This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, a written solicitation will not be issued. The solicitation number is 2011-99251. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-46. The associated North American Industrial Classification System (NAICS) code for this procurement is 334516 with a small business size standard of 500 employees. Proposals are due by June 30, 2011 at 12:00 p.m. The place of performance is Centers for Disease Control and Prevention, 1600 Clifton Road, Building 1, Atlanta, GA 30359. The approximate delivery date is August 31, 2011. The Centers for Disease Control and Prevention (CDC) intends to issue a sole source, firm fixed price contract award to Applied Biosystems, 850 Lincoln Centre Drive, Foster City, CA 94404, for a quantity of 200 of Part number 4342253, FG, MFC, GENE-EX, CONF4. This procurement will be processed under the authority of FAR 13.106-1(b)(1). Only one responsible source will satisfy the Agency's requirements because this vendor has already created a platform for the designed card that is required so they match the pre-existing cards for study comparison purposes. This notice of intent is not a request for competitive quotes/proposals, however, all responsible sources may submit a capability statement with a cost proposal and a completed copy of the provision at 52.213-3, Offerors Representations and Certifications - Commercial Items which, if received by the response date, shall be considered by the Agency. A determination by the Government not to open the requirement to competition based upon the responses to this notice is solely within discretion of the Government.

The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation - Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisitoin and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition and a statement regarding which, if any, of th eadditional FAR clauses cited in the clause are applicable to the acquisiton. 6.
