Florida International University Herbert Wertheim College of Medicine Community Outreach Consultant The Community Outreach Consultant will engage community organizations and individual women to offer and facilitate access to the Linda Fenner 3D Mobile Mammography Center (LFMMC) screening mammograms and breast health navigation services. The Community Outreach Consultant will be for $18,000 @ $2,250 per month from November 1, 2022, through June 30, 2023. The Community Outreach Consultant will submit the invoices monthly. The Community Outreach Consultant must complete the following services and dates by the dates referenced on this Scope of Work starting on 11/01/2022 as follows: 1. Collaborate with the Mobile Mammography Center team by communicating regularly and attending monthly team meetings. 2. By 6/30/2023, conduct five outreach sessions with the public at large, focusing efforts at the "grassroots" level with women who reflect the cultural diversity of Miami-Dade County, including at grocery stores, beauty salons, and places where eligible uninsured women might gather. 3. By 6/30/2023, meet in person or virtually with a minimum of five potential community partners, including places of worship, health and social service organizations, and businesses. Facilitate at minimum three new agreements with organizations that agree to refer women to the Mobile Mammography Center services by introducing these organizations to the Clinical Director of Operations for a partnership agreement. By 6/30/2023, conduct ten breast health education sessions (live or online) with existing partners (five sessions) and with new partners (five sessions). 4. By 6/30/2023, promote and educate the community on the Mobile Mammography Center breast health care service. (flyers, social media, broadcast media) at a minimum once every two months. 5. The Community Outreach Consultant will work with the Herbert Wertheim College of Medicine (HWCOM) Director of Media and Community Relations on these initiatives. The Department of Humanities, Health, and Society will pay for media-related expenses. 6. By 6/30/ 2023, facilitate Mobile Mammography Center services for 80 eligible uninsured women. 7. Submit monthly progress reports to the Chief Administrative Officer, Lead Physician, and Clinical Director of Operations—HWCOM Department of Humanities, Health, and Society. The monthly reports will include progress on deliverables.
