This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

This combined synopsis/solicitation is for a Non-Personal Service Contract for a Contractor to provide Family Nurse Practitioner/Physician Assistant (FNP/PA) Providers to the Urgent Care/Emergency Room Department at the Chinle Comprehensive Health Care Facility (CCHCF), Navajo Area Indian Health Service (NAIHS) throughout the duration of the contract period of performance, which is from Date of Award through 10/31/2017, with the option to extend to 12/31/2017.

I will accept offers from vendors who do not already have a contract with CCHCF for this need.
