This is a personal service contract at the Indian Health Service (IHS) Ft. McDermitt Health Clinic, 112 North Reservation Road, McDermitt, NV 89421-0457, who will act as the Diabetes Wellness Program Coordinator for the Indian communities of McDermitt, Summit Lake, and Winnemucca, Nevada for the period August 1, 2009 through July 31, 2010. The Wellness Coordinator shall develop, initiate, and oversee an activities program for the communities. Jogging, walking, dance, and other aerobic activities shall be promoted to stimulate increased emphasis on the potential diabetic.
