To provide the Warm Springs Service Unit with an annual Emerald Service Agreement for the X-Unity-80-00 and the E7869X X-Ray tube in the Radiology Department. (Maintenance Support Agreement) POP: 4/3/2015 - 04/2/2016.

Replacement of IIP H/W to FDX H/W. Will include replacement of old monitor with a new touchscreen monitor; upgrade of all FUJI CR reader software to the lastest and greatest. Upgrade to FDX workstation software to the latest and greatest. Will provide a PM on existing CR readers as part of the agreement. Upgrade MS Operating software from the EOL XP platform to Win 7.

1 application training day: necessary to train the end users on the new Software the FDX consoles will be running.

MSA to include all replacement parts, labor, travel, preventative maintenance inspection, calibration, and software updates. Includes on-site labor for installation, implementation and software upgrades for products purchase. Installattion.

Technician or Engineers must specifically be trained and certified to service the FUJIFILM equipment.
