Quotations are due on or before September 23, 2009 by 2:00 PM Central Standard Time to be considered responsive.ITEM 1: Catholic Priest to perform the services listed below in accordance with this contract at James H. Quillen VA Medical Center, Mountain Home, TN. To include the following: Weekly Mass/Service of the Word to be held on Tuesdays unless rescheduled a week in advance. Estimated weekly Masses/Services is 52 EA ; Provide Rite/Sacrament at bedside on a weekly basis, normally on Tuesdays unless rescheduled in advance. Estimated Rites/Sacraments is 780 EA; Provide patient visitation; general visitation of Catholic patients desiring visits from the contract Catholic Priest on a weekly basis, normally on Tuesdays unlessrescheduled in advance. Estimated number of patient visits is 260 EA; Provide Assessment Consultation for patients and residents who are Catholic and present complex religious and spiritual needs. These assessments will be completed within normal duty hours. Estimated consultations is 52 EA. Provide emergency call back which occur outside of normal duty hourswhen a patient or resident is placed on Seriously Ill status or has diedand requires the presence of a Catholic Priest.
