This is a Market Survey synopsis to identify qualified Small Business Concerns to submit capability statements of their qualifications for a recurring requirement for Mortuary Services at the National Institutes of Health within the Clinical Center. The North American Industry Classification System (NAICS) code is 812210. This is not a request for proposals or quotes and in no way obligates the Government to award any contract. The services to be provided are various mortuary and autopsy services for patients in the Clinical Center. These commercial services are required for both adults and children to include the picking up of bodies; preparation of the body, embalming, cosmetics, dressing of the body, placement in casket, providing caskets procuring all necessary local permits/certifications, cremation services, formal services, flowers, appropriate personnel to conduct services, local burial plots, domestic/foreign shipment of bodies. In submission of the vendors capability statement the following questions shall be addressed: 1) Technical capabilities; 2) Description of facilities in relation to preparation of bodies, visitation of bodies, chapel/church services and transportation, parking and adequate vehicles; 3) Personnel – resumes which include related work experience and; 4)Company experience/qualifications of comparable mortuary services. Information provided by the vendor(s) in response to this Market Survey shall not be disclosed if doing so would jeopardize the vendor(s) confidential information and shall be protected under FAR Part 3.104.
