This is not a Request for Proposal (RFP) or Request for Quotation (RFQ).
In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: To have available to the PIMC a instrumentation system that performs fully automated, on-demand molecular testing for the presence of Neisseria gonorrhea, Chlamydia trachomatis (GC/CT), Human Papillomavirus (HPV) and HPV genotyping from The ThinPrep PAP TEST and urogenital (endocervical and urethral) specimens; in a self-contained instrument environment. This testing service will be provided to all departments and outpatients clinics of the PIMC. (Refer to attachment A for additional information.)
Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from May 5, 2015 through May 15, 2015.
