This is a combined synopsis/solicitation for a Contractor to provide On-Site Geriatric Service at the Chinle Comprehensive Health Care Facility. Chinle, Az. This combined synopsis/solicitation is issued as a Request for Quote. NAIHS CODE: 621210. The requirement is for a firm fixed price (FFP). The Standard Form 1449 is included with this announcement.
The Contractor shall provide on-site Geriatric Dentistry Services as scheduled by Chinle Dental Department. (Fabricating and processing denture services). The Contractor shall provide all management, supervision, labor, materials, supplies, equipment, and transportation. Period of Performance: March 21, 2011 through September 30, 2011. Services will take an estimate of 4-5 trips to the Chinle Hospital to complete the services. The rates shall be inclusive of all necessary and associated costs, (i.e. travel, per diem, lodging, rental car, applicable taxes, One lab assistant, etc.), to complete the task.

Award shall be made at a fair and reasonable price to the responsive and responsible offeror whose quote conforming to the requirements herein will be the most advantageous to the Government and whose quote represents the best value as defined by FAR 2.101. In determining best value, the following factors will be considered: Past performance, Price and prior IHS experience. The Contractor should have training in Geriatric Dentistry and have expertise at fabricating full, upper, lower and partial dentures As part of the evaluation process, candidates may be interviewed to ensure their understanding of the scope of work and to verify their qualifications to perform required services. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value.
Background: IHS is an agency within the U.S. Department of Health and Human Services and is responsible for providing federal health services to American Indian and Alaska Natives. The provision of health services to members of federally recognized tribes grew out of the special government to government relationship between the federal government and Indian tribes. The IHS is the principal federal health care provider and health advocate for the Indian people. The goal is to assure that comprehensive culturally acceptable personal and public health services are available and accessible to American Indians and Alaska Natives. The IHS currently provides health services to approximately 1.5 million American Indians and Alaska Natives who belong to more than 557 federally recognized tribes in 34 states.
