The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF), Dental Department is requesting for submittal of offers for a Non-Personal Service Contract for a contractor to provide Geriatric Dentistry Services from 04/01/2017 through 9/30/2017. Services shall be performed in strict accordance with the attached Performance Work Statement (PWS) and Scope of Work (SOW).
