The Contractor must be responsible for coordinating all agreed upon activities of the Latino Health Initiative (LHI) Community Health Promoters Program. The Contractor must design, develop, train, implement, monitor and evaluate LHI’s Community Health Promoters Program. The Contractor will participate in periodic meetings with LHI Community Programs staff, who shall approve the Community Health Promoters Program project design, training, and implementation methods. The Contractor must design a culturally and linguistically appropriate Health Promoters Community Outreach Program Plan to be approved and implemented within the agreed upon timeframe as identified by the Montgomery County LHI Community Programs team. The Contractor must identify, recruit and maintain a team of a minimum of 20 Spanish-speaking health promoters to provide service to the community under the guidance of LHI staff. The Contractor must ensure that the health promoters are trained in the delivery of culturally and linguistically appropriate health promotion services based on the needs of LHI. The trainings provided by the Contractor must provide health promoters with advanced knowledge of County and community resources. The Contractor must also provide training to health promoters regarding roles and responsibilities, community outreach skills, health promotion topics such as healthy eating and physical activity, health and safety protocols, data collection tools, providing navigation support services, and in personal and individual self-care.
