This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is governed by the Buy Indian Act, 25 U.S.C. 47, and offers from Buy Indian small businesses only will be considered for award.

The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for submittal of offers for a contractor to deliver, assemble, install the various medical equipment. CCHCF has identified a specific brand name, but will accept brand name or equal descriptions per FAR 6.302-1(c)(2) that meet the required Specifications and Salient Characteristics. CCHCF intends to award a firm fixed price contract(s) as a result of this solicitation.

This combined synopsis/solicitation consists of three (3) parts. Refer to each Parts' respective Pricing Schedule and Specifications and Salient Characteristics:

 -Part 1: Exam Tables
 -Part 2: Vital Monitors
 -Part 3: C-Arm

CCHCF is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users.

EVALUATION CRITERIA:
The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Past Performance, Technical Capabilities, Specifications/Salient Characteristics, Delivery, and Warranty. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price.

Due to urgent need there will be no discussions regarding this solicitation. To be considered for award, your offer shall include the following documents:

1) Pricing: Complete and sign the attached Pricing Schedule. The Government will consider competitive rates upon negotiation.
2) Pricing: Provide your quotation.
3) Past Performance: Provide evidence in experience in providing services of similar scope and complexity to Federal Government or other large commercial/government entities within the last three (3) years of operation.
4) Technical Capabilities: Provide your product Specifications
5) Specifications/Salient Characteristics: Provide evidence that your offer meets 100% of the Specifications and Salient Characteristics
6) Delivery: Provide your Delivery date.
7) Warranty: Provide your Warranty terms.
