Face Mask Miscellaneous Requirements: 1. All masks must cover (Adult or Child) nose to under chin. 2. Bidders must provide a total cost per each. 3. Delivery must be made on January 7, 2021. Hand Sanitizer Requirments: Hand Sanitizer must: 1. be in a hand pump bottle. 2. be a pump bottle minimum size of a 8 ounces and a maximum size of 12 ounces. 3. be a minimum of 60 percent alcohol based. Hand Sanitizer Miscellaneous/Delivery Requirements: 1. Hand Sanitizer must adhere to the Food and Drug Adminisrtation safety and effectiveness of consumer antiseptic and topical antimicrobial drug products for over-the-counter human use. 2. Bidders must upload the number of ounces contained within the hand sanitizer pump bottle the bidder is quoting. 3. The District will accept hand sanitizer foaming or non-foaming. 4. Bidders must provide a total cost per ounce. 5. Delivery must be made on November 11, 2020. Disinfectant Wipes: Disinfectant wipes must be: 1. quaternary ammonium compound based. 2. a dwell time or contact time of 3 minutes or less kill claim for SARS - CoV2 COVID 19. 3. packaged in a minimum of 250 wipes per pack or pail. Disinfectant Delivery Requrements: 1. There will be two different deliveries for these items. The first 3 million disinfectant wipes order will be placed on October 21, 2020 and delivery must be November 11, 2020. The second 3 million disinfectant wipes order will be delivered January 7, 2021. Barrier Gowns: Barrier Gowns must be: 1. f ull-length barrier gown coated with a liquid impervious polyethylene barrier. Disposable, elastic cuffs, ties at waist. One size fits all. Barrier Gowns Delivery Requirements: 1. Delivery must be made on November 11, 2020. Miscellaneous Requirements: 1. Bid will be awarded by line item to the lowest responsive and responsible bidder. 2. All line item awards will be taken to the Columbus City Schools Board of Education meeting on October 20, 2020. If approved, orders will be placed for all the items on October 21, 2020. Please refer to the specifications for the respective delivery dates. 3. All deliveries will be made to the Columbus City Schools Hudson Warehouse, 737 E. Hudson Street, Columbus, OH, 43211 and must be able to deliver via lift gate. 4. Price per each and price per ounce must be inclusive of all cost. Bidders must provide samples, of their quoted product (s), within 24 hours of the request to be considered responsive. 7. When requested, bidders must provide samples and District must approve prior to an award. 8. Pallets must be 40 inches x 48 inches, four-way pallets with heights not to exceed 48 inches. 9. The deadline for submission of vendor questions is September 22, 2020 at Noon (EST). Questions must be submitted via Public Purchase. Questions will not be addressed over the phone or by email. Answers to vendor questions will be posted on Public Purchase by September 24, 2020 at Noon (EST).
