Sources are sought for service and maintenance of the Philips Forte Jetstream Nuclear Medicine Equipment and Workstation located at the VA Medical Center, 1500 N. Westwood Blvd, Poplar Bluff, Missouri 63901. Repondents must have a fully trained (certified by Philips) in-house technician, be able to obtain all parts, materials, and tools necessary to perform service and maintenance on the equipment (including 2 Preventive Maintenance Services annually), be able to respond via telephone within two (2) hours of the initial notification for for diagnostic response service and be on the premises within twenty-four (24) hours of notification. Respondents are requested to provide up to three past performance or current references for verification.
