Description of Services Required: Complete annual calibration of all Biomedical test equipment at the Tomah VA. Provide on-site (and depot as required) testing, adjustments/calibrations, preventative maintenance (PM) as required, certifications and documentation of all Biomedical test equipment. Services to be completed in May of each year of the contract (base plus 4 options). Ensure all testing, adjustments and calibrations on Biomedical shop test equipment are performed in accordance with manufacturer documentation/recommendation/specifications and meet applicable codes. The Contractor shall perform an annual on-site visit, on dates acceptable to the medical center and coordinated through the COR, to ensure all Biomedical test equipment (tentative list included above) are performing in accordance with manufacturer recommendations and all other applicable codes and standards. The on-site service shall take place in May of each year during the duration of this contract. Dates/times shall be scheduled with the COR. b. All travel, labor, report documentation, and calibrations are included in this contract. c. PM shall be performed on applicable equipment where required and include a 12 month warranty. d. The Contractor's Service Technician shall document the work and provide the results to the COR both in hardcopy and electronic form. Documentation shall identify each piece of test equipment (manufacturer, part number, model, serial number, etc) and standards utilized to conduct the calibrations. PM performed shall be annotated accordingly. All applicable documentation shall be provided to the COR, including individual calibration certificates for each piece of equipment.
