THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. No solicitation will be issued. This is a notice of intent to award a sole source contract in accordance to FAR 6.302-1(a)(2), authorized or required by statue. This is a one-time procurement. The VISN 10 Network Contracting Office intends to award a sole source procurement with ScriptPro to purchase: 1.0 GENERAL Contractor shall provide personnel to complete the task of providing full service to our ScriptPro Pharmacy Automation System at the Chillicothe Veterans Affairs Medical Center. (Site I.D. #5477) Full service is to be provided for each component and accessory of the ScriptPro Pharmacy Automation System. Including: SP 100/OCC SP 200/CCC (12 slot) SP Central Expanded Platform w/Station SP Station (Qty 9) SP Checkpoint Robotic Cell Multiplexor (Qty 7) Electronic signature Device Notice Board XL (Qty 2) Virus Protection Interface Upgrade (Platform) ScriptPro Inventory Management Module Scanner (Qty 2) Interface & Data Conversion Add-ons (Qty 1) 1.1 CONTRACTING OFFICER'S REPRESENTATIVE (COR) and TECHNICAL REPRESENTATIVE The Contracting Officer's Representative (COR) and/or Technical Representative will be the principal liaison for the Contractor during the duration of the contract. 2.0 SPECIFIC TASKS A) Provide both planned maintenance and emergency service. B) Provide all repairs and calibrations to the instruments. C) Provide all parts and labor (and return shipping costs if needed) associated with the repair and/or calibration of these instruments as incurred during this contract. SUPPORTING INFORMATION 1. Service Employee Qualifications: Service Employees shall be competent to perform the specific repairs. 2. Hours of Work: Contractor employees will work during normal business hours, 8:00a- 5:00p M-F. 3. Location of Work: All work will be performed in the Chillicothe VA Medical Center Pharmacy, 17273 St. Rt. 104, Chillicothe, Ohio 45601 4. System Requirements: Access to the ScriptPro Pharmacy Automation System and its components/accessories will be made available by V.A. personnel. 5. Privacy/Release of Information a. Personnel Clearances and Privacy Act Considerations. Contractor employee shall be required to observe the requirements imposed on sensitive data by law, Federal regulations, VA statutes and policy, and associated requirements to ensure appropriate screening of all personnel. Because of the inherent sensitivity of the data at the clinic facilities and data to be maintained at the facilities, the level of security clearance required of the Contractor's employee will reflect the same level of security clearance carried by the VA personnel for the function under contract. The Contractor shall ensure that its personnel meet the above restrictions and that confidential and proprietary information shall be divulged only to those officers and officials of the VA who are authorized to receive such information. b. All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start date of the contract, the contractor will be responsible for the actions of those individuals they provide to work for VA. 1. Position Sensitivity - The position sensitivity has been designated as Low Risk 2. Background Investigation - The level of background investigation commensurate with the required level of access in National Agency Check with Written Inquiries 3. Contractor Responsibilities - a. The contractor shall bear the expense of obtaining background Investigations, if the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 30 days. b. The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are permanent resident and are able to read, write, speak, and understand the English language. c. The contractor shall submit or have their employees submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt: (i) Standard Form 85P, Questionnaire for Public Trust Positions (ii) Standard Form 85P-S, Supplemental Questionnaire for Selected Positions (iii) FS 258, U.S. Department of Justice Fingerprint Application Chart (iv) CA Form 0710, Authority for Release of Information Form (v) Optional Form 306, Declaration for Federal Employment (vi) Optional Form 612, Optional Application for Federal Employment d. The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the contract. e. Failure to comply with the contractor personnel security requirements may result in termination of the contract for default. 4. Government Responsibilities - a. The VA office of Security and Law Enforcement will provide the necessary forms to the contractor or to the contractor's employees after receiving a list of names and address. b. Upon receipt, the VA Office of Security and Law Enforcement will review the completed forms for accuracy and forward to OPM to conduct the background investigation. The VA Facility will pay for investigations conducted by the Office of Personnel Management (OPM) in advance. In these instances the contractor will reimburse the VA facility within 30 days. The VA Office of Security and Law Enforcement will notify the contacting officer and contractor after adjudicating the results of the background investigations received from OPM. c. The contracting officer will ensure that the contractor provides evidence that Investigation have been completed or are in the process of being requested. The procurement will be for the VAMC Cincinnati, Ohio. The estimated amounted is $230,144.56. The Government believes that ScriptPro is the only company that can satisfy the agency requirements. Therefore, the Government intends to proceed with sole source procurement for the required equipment. To the best of our knowledge ScriptPro is the only source that provides the above indicated services. The North American Industry Classification System (NAICS) code for this acquisition is 334111. This is not a request for proposals; however, an interested party may e-mail a written capability statement for these required items, which shall not exceed two (2) pages in length. Interested parties may submit their capability statement, which may or may not be considered by the agency via email to [email protected] no later than 1:00 PM EDT on September 11, 2015. If no responses are received, the Cincinnati VA Medical Center will proceed with sole source procurement with ScriptPro.
