Service Agreement for Olympus Scopes This is a combined synopsis/solicitation for commercial items identified below, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: offers are being requested and a written solicitation will not be issued. This solicitation number: VA-263-09-RQ-0104 is issued as a Request for Quotation (RFQ), and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-29. This procurement shall be solicited under the Simplified Acquisition Procedures (SAP). The North American Industry Classification System Code (NAICS) code is 811219. The Department of Veterans Affairs, VISN 23, VA Medical Center, Sioux Falls, South Dakota, has a requirement for a 12 month service agreement to maintain an operating inventory of Olympus flexible colonoscopes, bronchoscopes, gastroscopes, and cyctoscopes used in this facility for medical procedures per the following SOW: REQUIREMENTS: The VA Medical Center, Sioux Falls, requires the following for consideration for this proposed contract: 1. Only original OEM parts (Olympus) will be used in repairs of Olympus scopes. The parts themselves must be authorized /purchased from Olympus for use. Any parts purchased from other sources, is strictly prohibited, and will not be acceptable. 2. The vendor must be Olympus factory trained to do any repairs, upgrades, calibrations, refurbishments, adjustments or testing of the scopes. 3. The vendor must complete all repairs within 30 days of receiving the scope from our facility. If repairs are expected to go beyond this period, a loaner of equal model and series will be provided to us at no charge. The loaner may be used by our facility until the scope has been repaired and returned to our facility. 4. The vendor must perform a site visit to assess the condition of each scope and to make an accurate count of total scopes to be included on the list as well as noting the correct serial number and model number of each scope covered on the contract. 5. The desired start date of contract coverage is 01/1/2009. 6.
