This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NICHD2010072, and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-38. The North American Industry Classification (NAICS) Code is 325414 and the business size standards are 500. However, this solicitation is a not a set aside for small business. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) intend to procure a Primary Care Service Contract from GE Healthcare Bioscience, 800 Centennial Avenue, Piscataway, NJ 08854-3911, for the following pieces of equipment: GE Healthcare Manufactured Voluson E8 Consoles - Qty (5) five each System IDs: 1. VOLE8_TBD2. VOLE8_TBD23. VOLE8_TBD34. VOLE8_TBD4 and5. VOLE8_TBD5 Period of performance: Base year (one full calendar year) plus three (3) option years. To the best of the Governments knowledge GE is the only authorized source able to provide the needed and proper service contract for the Voluson ES Console systems. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications Commercial Items with its offer.
