The Bath VA Medical Center located at 76 Veterans Avenue, Bath, NY 14810 has a requirement for six year preventative maintenance services on 6 Carefusion LTV-1200 ventilators. The preventative maintenance includes the following tasks completed by factory trained technicians, only using new original equipment manufacturer parts: " Replace the Turbine Manifold Assembly. " Replace the Solenoid Manifold. " Replace the Flow Valve. " Replace the Rotary Switch Assembly. " Replace the O2 Blender. (LTV® 1200 only) " Replace the Fan Assembly. " Replace all Silicone Tubing. " Check the Thermo Pads for compression and replace if necessary. The NAICS Code is 811219. In accordance with FAR 6.302-1, and because this item is unique and the only service able to work for the Bath VA Medical Center, it is the intent of the Government to sole source this contract. The expected cost range is below $25,000.00. The contract is expected to be awarded to Carefusion 203 Inc. 17400 Medina Road, Suite 100 Minneapolis MN 55447. The company is the only company that can provide this particular service to the Bath VA Medical Center. The set aside status is unrestricted. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the Bath VAMC.
