Statement of Work Dialysis Box Design Build For 16 Stations First Floor Dialysis Clinic Bldg 1, JC 1. Provide design build services for the removal and installation of 16 new dialysis boxes in the dialysis clinic, first floor, building 1, John Cochran. The new boxes shall be constructed of none porous composite material, gray in color, shall not promote bacterial growth, ergonomic to provide ease of equipment hook up, sterilization, flushing of RO supply line, sampling, and waste inspection. 2. The piping shall be replaced from the one inch supply loop tie-in valve to the dialysis machine connection. Provide an additional shut off valve easily accessed at the front of the box and quick connects from both the RO water supply line to the dialysis machines and the return waste line to the box connection. Waste return shall allow for visual inspection of the return waste fluids. The new boxes shall be sloped to drain quickly to shed water and minimize staining. Meet the Following Standards: a. Procedures will be followed to prevent the transmission of bacteria and endotoxin to hemodialysis patients. b. Dialysis water and Dialysate will be tested monthly. c. Product water used to prepare dialysate or concentrations shall not exceed 200CFU/mL and an endotoxin concentration shall not exceed 2EU/mL. d. The action levels for total viable microbial count in the product water shall be 50CFU/mL, and the action level for the endotoxin concentration shall be 1 EU/mL. 3. If the total viable microbial counts exceeds 200 CFU/mL for any given hemodialysis machine, the machine in question will immediately be pulled from service. Disinfection with heat and bleach BACT should occur as outlined in this document. 4. The existing stainless steel boxes shall be removed and new boxes installed in the same location. The installation of the new boxes shall result in minimal alterations to existing finishes, RO supply line, and waste plumbing. Upon completion of the installation all existing finishes shall be returned to as new condition. 5. The design build team shall be responsible for establishing means and methods for RO water system cleaning and start up including independent sampling and lab testing. The system shall meet the latest ANSI/AAMI standards for bacteria and LAL limits. 6. The design build team shall establish recommended criteria for system cleaning, flushing, sampling, and testing after the system, including individual stations, have been inactive for a period longer than 24 hours and/or bacteria or LAL counts exceed AAMI standards. 7. The system shall be in compliance with the following reference: Reference Drawings: 1-AS1.4b 1-AS2.2b 1-AS2.3b
