Preventative Maintenance (PM) Program for
FUJIFILM VisualSonics Vevo 3100 Ultrasound
Scope:
Preventative Maintenance:
The Contractor shall provide annual preventative maintenance services to keep the equipment in such a condition that it may be utilized at its original design capacity and efficiency. The contract shall cover all parts, labor and travel for all preventative and/or emergency repair services. The following areas shall be checked under the PM program:

Unscheduled Maintenance/Emergency Service and Repair:
The contractor shall provide the COR and/or designee with a phone number and e-mail contact available 24 hours a day, 7 days a week for the purpose of emergency repair.
The Contractor shall respond within 24 hours of initial contact by the COR or designee.
The Contractor shall be on-site for equipment assessment within 48 hours after initial contact placed by the COR or designee for emergency service.
The Contractor shall be allotted 24 hours after assessment to begin repair of equipment.
All overtime labor and travel related to emergency repairs will not be reimbursed by the Government and shall be included in this contract.
Equipment must be such a condition that it may be utilized at its original design capacity and efficiency at the end of the contract. Any costs related to repair or services still pending by the Contractor after contract end will be the responsibility of the Contractor.
The Contractor shall provide phone and e-mail-based technical and application support during normal business hours, 9am-5pm EST.

Period of Performance:
The expected base period of performance begins at time of award (and is considered the Commencement Date ) with two (2) Twelve (12) Month Option Periods that may or may not be exercised based on the needs of (and at the discretion of) the Government. Exercising of the option years shall only be done by the Contracting Officer.
Type of Contract:
A Firm Fixed Price Contract is requested for this requirement.
Information Security:
In accordance with Appendix A, Handbook 6500.6 Checklist blocks 7, the C&A requirements do not apply to this requirement, and Security Accreditation Package is not required. The nature of this requirement is technical; however, there is no information to be protected.
