Provide accreditation and proficiency testing to the laboratory located at the Rapid City Service Unit to cover CLIA 88 regulations. Period of Performance is November 19, 2016 through November 18, 2017.  Bendor will provide all testing materials, evaluate test results, furnish reports to Rapid City and CMS as needed, conduct on-site inspections, provide Continuing Education Units and maintain CLIA deemed status, and issue accreditation certificate.
