Provide accreditation and proficiency testing to the laboratory located at the Rapid City Service Unit to cover CLIA 88 regulations.  Period of Performance is 17 November 2015 through 16 November 2016.  Vendor will provide all testing materials, evaluate test results, furnish reports to Rapid City and CMS as needed, conduct on-site inspections, provide Contining Education Units and maintain CLIA deemed status, and issue accreditation certificate.
