This is a combined ALL or NONE Basis synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Northern Navajo Medical Center, Navajo Area Indian Health Services is requesting for submittal of offers for a contractor to provide Laparoscopic Supplies to the Operating Room Department On an As Need Basis BPA. Per FAR 6.302-1(c)(2), NNMC has identified a specific brand name, but will accept brand-name or equal.

EVALUATION FACTORS
The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: Quality/Features, Warranty, Delivery Terms and Past Performance.

The quotes will be evaluated based on these three factors from a rating scale:
1. Price
2. Quality/ Features of the request
3. Delivery Terms

In the event of a tie, tie breaker will be price.

The contracting company shall submit the following ALL or NONE quote with their offer;
- Evidence of Past Performance / Capability Statement
-Socioeconomic status
- Quote with POC, acceptance of PO.
- DUNS #
-Item descriptions or commercial specifications/use


To be considered for award please provide a quote with all submittal requirements before the solicitation closing date and it must be submitted via email. Advance payment is not approved for this request. Therefore payment will be rendered until after delivery of items and an invoice is submitted within 30 days.

Quotes submitted after closing date will not be considered.
