This Request for Quotation (RFQ) is being issued via solicitation number RFQ18-3021.

The Navajo Area Indian Health Services, Shiprock Service Unit (SRSU) Northern Navajo Medical Center (NNMC) Operating Room (OR) Department is requesting for submittal of offers for a contractor to provide a "Brand Name" or "Equal" Laparoscopic SurgicalMesh.

Resultant contract(s) will be on an As Needed Basis from Date of Award through (1) year.

EVALUATION FACTORS:
The Government reserves the right to issue a single award or multiple awards to schedule contractor(s) whose quote represents best value as defined by FAR 2.101. In determining best value, the Government reserves the right to select the most advantageous offer to the Government by evaluating and comparing initial quotations. The evaluation factors and significant subfactors when combined are significantly more important than to cost or price. The socioeconomic status of an offeror shall also be considered, should one or more quotes represent best value.

Factor 1: Technical Capabilities: Provide evidence that offered product meets the technical capabilities of attach salient characteristics of product.

Factor 2: Delivery Terms:
Subfactor: FOB Destination
Subfactor: Delivery Time (2-3 Day or sooner)

Factor 3: Past Performance: Provide evidence of providing Laparoscopic Mesh to Federal Government or other large commercial/government entities within the past three years of operation.

REQUIREMENTS FOR SUBMITTAL OF OFFER:
