Requirements

Remove and replace pre-determined parts - The contractor must be a qualified service technician authorized by the manufacturer to perform the required maintenance and/or repair. Maintenance and/or repair period must meet but are not limited to the following requirements:

The contractor must furnish the following parts:

- Two (2) Refurbished ULR-75 Lasers (Part Number: 130-0154-R0-A)

- One (1) Honeycomb Insert ILS12 (Part Number: 700-1639-00-A)

 The contractor will be responsible for the removal and installation in accordance with the manufacturer's specifications and procedures

The contractor must perform a post maintenance functions check to ensure the ILS12.75 is fully operational and can be utilized safely.

All maintenance/repair tasks must be completed by the contractor on site at 17001 Interstate Drive, Suite 140, Dumfries, VA 22026, Monday - Friday between 6:00am - 4:30pm

Provide on-site training for 5 NMMC staff members - The contractor must be a qualified trainer authorized by the manufacturer. Prior to on-site training, it is the contractor's responsibility to ensure the ILS12.75 is 100% fully operational Training must include but is not be limited to:

o Proper operation of the ILS12.75 utilizing all manufacturer's designed machine features

o Proper operation utilizing all materials listed within the manufacturers designed machine capabilities

o Proper operation utilizing of the ILS12.75 rotisserie attachment

o Basic end-user maintenance procedures on the ILS12.75

 The contractor shall utilize the NMMC's ILS12.75 Universal Laser during on-site training. It is the contractor's responsibility to provide any literature required for instructional purposes

 All training tasks must be completed by the contractor on site at 17001 Interstate Drive, Suite 140, Dumfries, VA 22026, Tuesday - Thursday between 8:00am-2:00pm
