You are invited to submit a Request for Quote (RFQ) to the referenced solicitation.

The Phoenix Indian Medical Center (PIMC) has a requirement for Laundering Services to be provided for the PIMC, located in Phoenix, Arizona.

Standard Form (SF) 1449, Solicitation/Contract/Order for Commercial Items, requires completion of section nos. 12, 17, 23, 24, & 30.

Price Quote must be filled out on Page 3.

Offerors must complete Pages 20-37.

Offerors are required to submit required documents on Page 46 - 49.

All sections must be completed otherwise the IHS will not consider your RFQ.

Complete proposal are due by 12:00 P.M., Arizona Time on June 07, 2013.
