" Company Name, Address, Phone Number, Point of Contact " Three Reference Points of Contact for similar work " Previous Hospital/Medical Clinic Project Experience within last three years The Contractor shall provide professional urology services. The service includes diagnostic and treatment of VA patients with illness or injury to the genitourinary tract; that is the genitalia, urinary tract, bladder, prostate and kidneys. The services will include the full range of urological services for surgical or medical patients. The service also includes a Urologic Clinic three days a week. Some procedures cannot be performed on station and will require the Urologist to see patients in Reno at a local facility. The on-call and consultation service is provided 24 hours per day, as necessary, seven days a week.
