CITY OF MONMOUTH MADRONA SIDEWALK PROJECT Bids due 2:00 pm, April 14, 2022 ADVERTISEMENT FOR BIDS Sealed bids for the furnishing of all labor, materials and equipment necessary to complete the "Madrona Sidewalk Project" will be received at the office of Russ Cooper, City of Monmouth Public Works Director, 401 Hogan Road, Monmouth, Oregon, 97361 until April 14, 2022 at 2:00 pm, (PDST). The bid opening will follow immediately thereafter. Proposals shall be clearly marked with the project title, showing date and time of bid opening, and with the name and address of the bidder. All proposals received will be publicly opened and read aloud at the above place, time and date. Bids received after the above date and time will not be accepted. Bids must be written on the forms provided in the Contract Documents and must be signed by the Bidder, or an authorized representative. A non-mandatory pre-bid meeting will be held April 7, 2022 at 2:00 at the project location listed below. This project consists of the following: Infill of missing sidewalk and drive ways from the intersection of Madrona Street E/99W to the intersection of Madrona Street E/Atwater Street S: • Excavation for construction of new sidewalk and associated driveways • 25 mailbox relocations to rear of new sidewalk • Adjustment of water meters to new grades due to new sidewalk construction • Construction of 1584 LF of 5' wide side walk (4" min. thickness) • Construction of 9 dual ADA ramps • Construct 34 LF of new Type C Curb • Remove and replace 13 residential driveways (4" min. thickness) All construction shall be completed and accepted by the City by June 15, 2022. This includes construction and any final correction items upon City of Monmouth final inspection. Copies of the contract documents are available from the City of Monmouth Public Works, 401 Hogan Road, Monmouth, Oregon 97361. At a minimum, the following plan room services will receive sets of Contract Documents for the work contemplated herein: Oregon DJC Plan Center, Portland OR; Salem Contractors' Exchange, Salem OR; Each proposal shall be accompanied by an unconditional certified check, cash ier's check or bidder's bond in the amount of five (5) percent of the total amount of the bid, payable to City of Monmouth, Oregon, as a guarantee that the bidder, if his/her proposal is accepted, will promptly execute the contract, secure payment of worker's compensation insur ance, and furnish a Separate 100% Corporate Surety Performance Bond and a Separate 100% Payment Bond for the faithful performance of the contract. The contractor may be asked to have their Surety furnish a letter certifying they have currently reviewed the Contractor's finan cial statement and that the Contractor is financially sound prior to award. Contractor must comply with all prevailing wage laws as required. Qualifications: Bidders must be an established business (minimum three years) doing like projects and be registered with the Oregon Construction Contractor's Board. References: Prime Bidders must present three (3) references, including business name, contact name and title, phone number and email address, for projects similar in size and scope and completed within the past five (5) years. References must be included in the bid packet. Failure to supply references will result in bid rejection. A subcontractor listing is required and can be submitted with the bid. If the subcontractor listing is not submitted with the bid, it must be received within two (2) hours after the bid closing time and date and can be emailed to dkarr@ci. monmouth.or.us, fax 503-838-0201, or hand delivered to 401 Hogan Road, Monmouth, Oregon 97361. Failure to supply a correct subcontractor listing may result in bid rejection. The Owner reserves the right to reject any Contractor who has not performed acceptably on past projects with the Owner, or others (this includes sub-contractor relationships). The Owner reserves the right to reject any Contractor who the Owner determines cannot do the project within the timeline or does not have the resources and financial stability to complete the project. The Owner reserves the right to postpone award for a period not to exceed 14 days after the bid opening; to reject any or all bids; to reject any bid not in compliance with prescribed solicitation procedures and requirements and other applicable laws; to waive any informality in a bid; to increase or decrease bid items depending on available funds; and to make awards in the best interest of the Owner.
