4. Maintenance Agreement

1.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued.
2. The solicitation number is 2015-81627 and is being issued as a request for proposal.
3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73.
4. This acquisition is for full and open competition. The North American Industry Classification System (NAICS) code is 334516 with a size standard in number of employees 500 (U.S.)
5. Performance is to begin August 15, 2015 and continue through September 14, 2015.
6. The provision at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition.
7. The provision 52.212-2, Evaluation-Commercial Items.
8. Offerors shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. (Note, if offerors are registered within the System for Award Management (SAM) and certifications and representations are up to date, make a statement to that affect in accordance with the provision.
9. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
10. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial items, applies to this acquisition and FAR clauses applicable are 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-40, 52.222-54, 52.225-13, 52.232-33, and 52.222-51. Other applicable clauses including Health & Human Services Acquisition Regulation (HHSAR), and CDC applicable clauses.
11. There are no special contract requirements or terms and conditions related to contract financing arrangements or warranty.
