The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for submittal of offers for a contractor to provide, deliver, assemble, install the various medical equipment. CCHCF has identified a specific brand name, but will accept brand name or equal descriptions per FAR 6.302-1(c)(2) that meet the required Specifications and Salient Characteristics. CCHCF intends to award a firm fixed price contract(s) as a result of this solicitation.
This combined synopsis/solicitation consists of three (3) Parts. Refer to each Parts' respective attached Pricing Schedules and Specifications and Salient Characteristics:
Part 1: Bariatric Bed & Lift
Part 2: Panda Warmers
Part 3: Ultrasound
CCHCF is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users.
EVALUATION CRITERIA:
The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Past Performance, Technical Capabilities, Specifications/Salient Characteristics, Delivery, and Warranty. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price.

1) Pricing: Complete and sign the attached Pricing Schedule. The Government will consider competitive rates upon negotiation.
2) Pricing: Provide your quotation.
3) Past Performance: Provide evidence in experience in providing services of similar scope and complexity to Federal Government or other large commercial/government entities within the last three (3) years of operation.
4) Technical Capabilities: Provide your product Specifications
5) Specifications/Salient Characteristics: Provide evidence that your offer meets 100% of the Specifications and Salient Characteristics
6) Delivery: Provide your Delivery date.
7) Warranty: Provide your Warranty terms.
