(ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price contract in response to Request for Quotation (RFP) 75H71019R00016. This solicitation is issued as a Small Business Set-Aside.
(iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-05.
(iv) The associated North American Industry Classification System code is 621910 and the small business size standard is $15.0 million.

(vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide ground transport services per RFP 75H71019R00016, SEE ATTACHED.
(vii) Date and Place of Delivery: Start date January 1, 2020, Kayenta Service Unit: Kayenta Health Center, Inscription House Health Center, Dennehotso Health Center and Four Corners Regional Health Center.
(viii) PERFORMANCE WORK STATEMENT: The government requires Inter-Facility and Intra-Facility Non-Emergent and Emergent Ground Transportation Medical Services for the Navajo Area Indian Health Service, Kayenta Service Unit and the Four Corners Regional Health Center. See RFP 75H71019R00016.
(ix) Evaluation Factors: This will be a best value procurement. Evaluation Factors: Past Performance, Technical Capability and Logistics. These non-cost factors are significantly more important than price. The Government will determine the reasonableness of each acceptable offer.
