The Indian Health Service, Gallup Service Unit, located in/at 516 E. Nizhoni, Gallup, NM 87301, has an upcoming requirement for three (3) Nurse Practitioners to perform on-site services to the eligible beneficiaries of the Indian Health Service. The North American Industry Classification System or NAICS code is 621111 with a small business size standard of $16.0 million. 

The Request for Quotations No. 75H71023Q00055 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13.5 Simplified Procedures for Certain Commercial Products and Commercial Services will be issued on or by June 7, 2023.  The anticipated period of performance is a base plus three (3) option periods contract. See below for full service performance:

Base Period:                07/01/2023 through 09/30/2023

1st Option Period:       10/01/2023 through 12/31/2023

2nd Option Period:     01/01/2024 through 03/31/2024

3rd Option Period:      04/01/2024 through 06/30/2024

Evaluation factors may include technical capability to meet the government need, minimum education requirements, professional certifications, past performance, and price. Quotes will be due on or by June 28, 2023, by 10:00 a.m. MDT. The acquisition is a total 100% Small Business set aside.
