The Albuquerque Area Indian Health Service (AAO) intends to award a firm-fixed price, non-personal service Blanket Purchase Agreement(BPA), commercial item in response to Request for Proposal (RFP) 16-242-SOL-00047.

This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1

The unit price must be all inclusive (to include but not limited to travel, lodging, per diem, fringe benefits, federal, state and local taxes) plus all other cost pertinent to the performance of this contract. Utilize your most competitive and reasonable rates.

The contractor will be required to perform for 8 hours per day x 3 days per week x 52 weeks = 1248 per contract year. The contract will be awarded as a single fixed price requirement contract for base year and four one year option years for renewal. The contractor must have a valid/current nutrition license in the State of New Mexico, be registered by the American Dietetic Association as a Registered Dietician Nutritionist, and be certified as a Diabetes Educator and experience in working at a facility.
