The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Service Purchase Order for Off-site Radiology Services for the Lower Brule Indian Health Center in Lower Brule, South Dakota. The IHS anticipates the award for one (1) base year plus two (2) option years. The period of performance for the base year will be from February 1, 2011 through January 31, 2012. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-11-071, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-11-071 and provide a postal mailing address, email address, and/or fax number and indicate how the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be December 30, 2010. The closing date of the RFQ package is January 14, 2011. This is a total small business set-aside request.
