3. BACKGROUND: The Assistant Surgeon General for Modernization, Office of Air Force Surgeon General (AFMSA/CC) has identified a need for an online Internet based PHR. These tools, usually available through the internet, can help people track their health care services and better communicate with their providers. The type of Personal Health Records AFMSA is envisioning are populated with health information from existing data warehouse managed and controlled by AFMSA. In the future, these records may be able to get information from a provider's electronic health record system, and some provider's may begin to allow patients to see the information directly from those electronic records. In general, a Personal Health Record (PHR) is controlled by the individual, and can be shared with others, including caregivers, family members and providers. This is different from a provider's electronic health record, which is controlled by the provider just as paper medical records are today. Ideally, a Personal Health Record will have a fairly complete summary of an individual's health and medical history based on data from many sources, including information entered by the individual (allergies, over the counter medications, family history, etc). Today, Personal Health Records are available from a number of sources: by many health plans for their members, by providers for their patients, and by independent vendors who are given permission by the individual to receive and store information from health plans, providers, pharmacies, labs, etc.
4. AFMSA vision is to implement PHR web services that allow customization from an existing product/service being used today from a vendor who has already penetrated the market and can be regarded as an industry leader with proven PHR performance/case studies. Included in this vision is to allow electronic health devices such as blood pressure monitors, pedometers, glucometers, and others to gather valuable information stored in one centrally, and then use it with tools that help individuals track health progress and manage fitness goals.
5. MANDATORY REQUIREMENTS:
• Vendor must have an existing web service PHR product/service and be considered an industry leader
• Vendor must have a software development kit (SDK) to allow customization of the a PHR presence and allow an automated data feed from AFMSA and/or DOD health data sources using approved data transfer protocols compliant to all privacy guidelines.
• PHR product/service must currently allow the seamless connection and automated data delivery via external home use bio medical devices such as wireless glucometers, weight scales, blood pressure devices, etc.
• Vendor must have growing partnerships with other healthcare related entities as an additional information offering, establishing a seamless online healthcare community of interest.

Information provided as a response to the RFI will not be returned.
