Item 0001: OPTEC 2300 Vision Tester, 6 Each Unit price and total price w/standard commercial warranty and service available in the delivery location identified in the quote.

This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a).  The North American Industry Classification System (NAICS) Code is 339115 and the small business size standard is 500 employees.  Classification Code is 65 – Ophthalmic Goods Manufacturing.

The selected Offeror must comply with the following commercial item terms and conditions. Provision 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. Provision 52.212-2, Evaluation – Commercial Items applies and the following evaluation factor will be considered to meet the minimum requirements of the government:

The contract will be awarded on the basis of best value to include Technical, Past Performance, and Price. 

-Technical Factor of meeting the requirements of the Brand Name Justification for the OPTEC 2300 Vision Tester

-Past Performance based on SPRS Supplier Risk Score is required to be no less than Green/Satisfactory.  No past performance/SPRS score will be rated as Neutral.

 -Lowest price that meets the minimum solicitation requirements and past performance.
