The objective of this sole source award is to procure AMO Sales and Service INC. Lenses, Signature Tubing and Patient Interfaces. The reason for the sole source is because of the requirement for consumable that works with the instruments and Johnson & Johnson equipment already purchased that the staff surgeons know how to use.

The notice of this intent is not a request for competitive proposals and no solicitation document exists for this requirement. This is also a highly urgent requirement for the facility and any information received based off this notice will be considered for future acquisitions.

JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION:

1. The Ophthalmology department is requesting the AR40E large lenses for cataract surgery. We need a three-piece lens to be used in emergency situations when a one-piece lens (ZCBOO) cannot be used.
Additionally, we need a consumable that works with the instruments and equipment already purchased that the surgeons know how to use.

2. The AR40E is the leading three piece lens on the market. It also works with the Johnson Vision consumables and instruments already purchased at Naval Hospital Bremerton. Lastly, our surgeons have been trained and are competent with how to insert lens.

3. Descrintion of market research.
After spending countless hours on the internet searching for three-piece lens and talking to the Johnson Vision customer service representative John, the current AR40E Large lens only works with the current consumables purchased and instruments already purchased by Ophthalmology.

4. Requesting four with each individually costing $128.00. Total or this item is $512.00.

1. The Ophthalmology department is requesting to purchase the signature tubing that is used specifically with the Whitestar Phacoemulsifier to exchange the fluid during the cataract Jlrocedure.

2, The signature tubing is patented by Johnson Vision and is the only consumable item that will work with the Whitestar Phacoenmlsifier.

3. After talking to the Johnson Vision customer service representative John, he re-integrated that the signature tubing is the only tubing that will work with our current equipment and no other company or supplier is authorized to sell that tnbing.

4. Ophthalmology is requesting two with each individually costing $336.00. Total for this item is $672.00.

5. We are requesting that these items arrive no later than 22SEP2017.
The Ophthalmology department is requesting to purchase interfaces for the Advanced Femtosecond Laser. During the procedure, the patient interfaces are used in conjunction with the Johnson Vision Laser to create a LASIK flap.

2. This item is the only item that can be purchased because it's the only consumable that is compatible with current laser and Johnson Vision is the only supplier who sells them.

3. Called Johnson Vision and spoke to customer service representative John who told me that this consumable is still the only authorized item for the laser and Johnson Vision is the only place that sells them.
