The Aberdeen Area Indian Health Service is currently seeking potential bidder's for a Firm Fixed Price Non-Personal Service Healthcare Purchase Order for a Optometrist at the Spirit Lake Health Center in Ft Totten, N.D. The period of performance will be from October 1, 2008 through September 30, 2009, with three (3) one (1) year options.  Contractor will work two (2) days per month. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state, and local taxes. Request for the RFQ package shall be in writing either by email or telefax and it will be made available approximately fifteen (15) days AFTER the date of this publication which will be August 7, 2008. The closing date of this RFQ is August 22, 2008.
